I confirm that my dog is in good health and fit to attend boarding, daycare, grooming, or agility sessions.*

I confirm that my dog is free from Kennel Cough and has not shown any symptoms (such as coughing, sneezing, or nasal discharge) within the last 14 days*

I confirm that my dog’s core vaccinations (including Kennel Cough if required) are up to date, and I can provide proof of vaccination if requested.*

I confirm that my dog has not been unwell or injured recently and has not been in contact with any other dogs known to be unwell.*

Veterinary Care: If your dog becomes unwell or injured while with Premier Paws, you (the owner) will be contacted immediately and are responsible for arranging and paying for any veterinary care. If you cannot be reached and urgent treatment is required, Premier Paws may seek veterinary assistance, and all costs will be the owner’s responsibility.*

I understand that my dog will be in a social environment and will mix with other dogs during their stay or session.*

I acknowledge that even well-behaved dogs can react differently in a group setting, and minor incidents, scuffles, or injuries can occur.*

I accept this as a known risk of social interaction and agree that Premier Paws cannot be held responsible for any injury caused by normal dog behaviour.*

I confirm that my dog has no history of aggression towards people or other dogs.*

If my dog shows signs of aggression or behaviour that poses a risk, Premier Paws may remove them from group activities or contact me for immediate collection.*

I accept that I am fully responsible for any injury or damage caused by my dog to another dog, person, or property.*

I have read and agree to the Behaviour, Health & Safety Agreement.

I understand my responsibilities as the owner and accept all related risks.

I give Premier Paws Ltd permission to act in my dog’s best interests in case of illness, injury, or emergency.

I consent to Premier Paws Ltd holding my contact details and using phone, email, or post to communicate when necessary.*
